* A 3% processing fee is added to all card swipes. This includes the Health Savings Account and Flexible Spending Account cards.
** A fee of $50 will be charged for returned checks due to insufficient funds.
Deposits for Appointments
• A New Patient Deposit of $175 must be paid at the time you schedule and fill out the New Patient Forms. This deposit goes towards the cost of the initial appointment.
• Return visits for established patients do not require a deposit.
New Patient Forms
• After making your appointment for your initial visit, New Patient Forms will be emailed to you for completion. Please complete all of the forms within at least 5 days of your initial visit. This gives us enough time to thoroughly review your information beforehand.
• Please submit any recent lab work (within 6 months) and/or other pertinent documents along with your New Patient Forms.
• If the paperwork is not completed within at least 5 days of your initial visit, your New Patient Deposit of $175 will be forfeited and the appointment will be cancelled. The appointment will need to be re-scheduled at a time when paperwork can be submitted with ample time to review it.
About Our Cancellation Policy, Re-Scheduling, and No-Show Appointments
• A notice of at least 48 hours is needed in order to cancel or change your appointment.
• The New Patient Deposit of $175 goes towards the cost of the New Patient Initial Visit or will be refunded if the appointment is cancelled or re-scheduled with at least a 48 hour notice. However, the full New Patient Deposit of $175 will be forfeited if:
1. New Patient Forms are not filled out on time (at least 5 days before the appointment)
2. You cancel or re-schedule less than 48 business hours before your visit
3. You fail to show up to your appointment (no-show)
• Return Visits: Although return visits for established patients do not require a deposit at the time you schedule those appointments, a fee of $90 will be charged to the patient’s credit card on file if:
1. You cancel or re-schedule less than 48 business hours before your visit
2. You fail to show up to your appointment (no-show)
Patient Status After Not Receiving Regular Care
• Any patients who have not received care in 3+ years are considered new patients. They will have to submit New Patient Forms and make an appointment for an initial visit.
Pricing
New Patient Initial Visit -
$375*
This price includes the amount of time spent reviewing the New Patient Forms and lab work before the initial appointment, the 90-minute initial visit itself, and the time spent creating your individualized treatment plan.
A New Patient Deposit of $175 must be paid at the time you schedule and fill out the New Patient Forms. This deposit goes towards the cost of the initial appointment.

Return Visits -
$45*/15 minutes
Each 15-minute time window is $45.
For example:
15 minutes = $45
30 minutes = $90
45 minutes = $135
60 minutes = $180
When you schedule a return visit online, a 1-hour time slot will be set aside.
Return visits for established patients do not require a deposit.
